Chaperone/Volunteer Information

Interested in Being a Chaperone or Volunteer?

All chaperones and volunteers are required to complete a Volunteer Consent Form and undergo a background check before

participating in any school events or field trips.

Please note: You cannot serve as a chaperone or volunteer until you have been cleared by the Office of Safety & Security.

The forms and step-by-step instructions are attached for your convenience. Since the approval process can take up to 5 business days, we recommend completing it at least two weeks before the event or field trip.

Security clearances for volunteers must be renewed every two years. Volunteers will be issued volunteer badges, which must be displayed at all times while on school property.

Types of Volunteers

Level 2 Volunteers:

(Supervised interactions with students)
 

Level 3 Volunteers:

(Chaperone for field trips, ride the bus, etc)
  1. ALL level 3 contractors/volunteers must have a Fingerprinting & Background Check request form completed prior to the background check appointment. (Completed by an APS Administrator)
  2. The applicant must bring the consent form to the appointment.
  3. An unexpired form of identification that contains a photo (i.e. driver’s license, state issued identification card, Permanent Resident Card, or a US Passport) is required.
  4. A $45 payment must be paid in advance
